The book is devoted to recommendations on reporting cases of near-miss in maritime transport. She emphasizes the importance of creating a blame-free culture to encourage open reporting of such incidents. The main goal is to stimulate the identification and elimination of potential hazards, as well as to comply with the requirements of the Safety Management Code (ISM Code). The book also calls on Member States and international organizations to inform all stakeholders about these recommendations.
