These forms are designed to simplify entry formalities in foreign ports and eliminate possible language difficulties. They are not intended to take the place of Registration Documents and/or Ship’s Papers which should always be available for inspection.

The details required should be inserted in the space provided and the language spoken in the port being visited indicated by a mark in the appropriate box.
The completed form can then be detached from the pad and presented to the immigration, customs or harbour offices as necessary.
